Subject:Strange screwups on on playback

I'm recording tapes into Sound Forge through a mixing
console to my PC. Lately, Sound Forge has been repeating
random parts of the recording. For example, if the audio is
a person saying: "I bought a house," on Sound Forge it
might playback as "I bought bought a house." Has anyone
else experienced this? Any ideas as to what's causing it?
