Subject:Wav file and Mp3 distortion

Does any body know how to solve my problem?

When I create wav files or mp3s, straight from a CD with
any program from CDEX, to Audio Catalyst to Adaptec, they
sound slightly distorted when played back through any
application. It's like a slight electronic clipping. I've
tried reducing the wave output from the volume control
window, installing new drivers but nothing seems to work!

Please Help!
