Crossfades not working as expected

I do a lot of automatic crossfades and I'm having a problem that I can't resolve.

My normal mode of crossfading is to always have the first clip slowly fade out while the second clip slowly fades in. At first, while assembling clips, I will get the desired effect. But at some point, all of a sudden, whenever I do a crossfade the first clip will fade to black while the second clip fades in.

The fade to black also seems to take precedence so the net result is that my edit appears to fade to black and then suddenly the second clip appears at full level with no apparent crossfade.

Why does this happen? How do I fix it?

Are you seeing this with rendered video, or just in preview mode? If it's just in preview, I'd suggest taking a sample clip that shows this and rendering just that region to DV AVI (or just to a new track), then take a look at the resulting video to see if the problem persists. Or do a RAM preview.

I've seen Vegas do some interesting things with transitions when previewing in order to keep up with things.

Did you check the second clip to make sure it had enoug overlap for the dissolve?

It does this both in preview mode and in the rendered video.

Also, it is important to note that the problem occurs at a time in the project after everything has been working fine for a while. I begin assembling clips with crossfades and they all look fine. Now, at some particular point in the editing, I drag a clip over another one and examine the crossfade. Not only has this paritcular edit gone into the bogus mode but ALL of my crossfades in the project have changed. I can't find any way to get them back to normal.

Just as a side note, my experience with Vegas has shown consistently that I can begin a project and start assembling clips and everything is going along fine until, all of a sudden, something happens and I am unable to return to the mode that I was previously in. Specifically, one of those occurences was a situation where the cursor behavior would change in the trimmer. It turned out that I had to check "Allow edit cursor to be dragged'" in the General Properties box. This still happens on a regular basis but now I know how to fix it. It seems to me that I am hitting some unknown hot key that changes me into the wrong mode and I simply have to find out what check box in what dialog gets me back. I really think that's what is happening here.

The question is how do I explicity set default crossfades to the style that I want?

If I understand your question, the answer is yes. In fact, I actually exaggerate the crossfade (make it 5 seconds instead of .1 second) just to see the effect clearly.
